Intraarterial thrombolysis in vertebrobasilar occlusion.
PURPOSE To report our experience using intraarterial thrombolysis in the treatment of vertebrobasilar occlusion. METHODS Twelve patients with 13 angiographically proved thromboses of the vertebrobasilar system underwent local intraarterial thrombolysis with urokinase. Angiographic and clinical outcomes were analyzed with respect to clinical examination at presentation, arterial occlusion patt...

The poor prognosis of basilar artery occlusion is well known. Systemic anticoagulation rarely prevents a lethal outcome. A new therapeutic approach involves selective perfusion of streptokinase through the vertebrobasilar artery via a coaxial catheter system. Three of five reported cases demonstrated successful vascular recanalization with clinical improvement.
متن کاملLocal intra-arterial fibrinolytic therapy in acute carotid territory stroke. A pilot study.
The possibility that intra-arterial local infusion of fibrinolytic agents may achieve recanalization of previously occluded carotid territory arteries in acute stroke was tested in a prospective angiography-based open pilot study at two centers. Fifteen of 20 patients with acute symptoms (mean treatment-onset interval 7.6 hours) demonstrated complete recanalization; 10 of the 15 patients exhibi...
متن کاملIntra-Arterial Thrombolytic Therapy for Acute Basilar Occlusion
Acute thromboembolic occlusion of the basilar artery accounts for 6% to 10% of large-vessel stroke in humans. Because of the brain region supplied by this artery, the case fatality rate is the highest for all ischemic stroke subtypes, ranging from 40% to 86%. Patients who undergo successful recanalization of the basilar artery by intra-arterial thrombolysis have lower mortality of 39%. Consider...
